Qualtrics Survey Tool

Qualtrics is a self-service cloud-based online survey, reporting and analysis tool made available to all UNTHSC faculty, staff and students. The intended use of the Qualtrics survey program is to collect information, judgments, or opinions. It should not be used for graded assessments of students. The survey creator will be considered the owner of the data gathered.

Qualtrics must be accessed through this web page to comply with our agreement, log in with your UNTHSC network ID and use UNTHSC templates. If you need login assistance, contact the UNTHSC Help Desk.

Qualtrics, Inc. provides extensive training, support (both technical and research), and it can provide survey panel respondents if requested. There would be a charge for Qualtrics to provide panel respondents, but all technical support and training charges are included in the UNTHSC contract.

Surveys by their nature are inherently research. Any survey that includes the personal information of any “human subject” may require prior review and approval of the UNTHSC Institutional Review Board (IRB). Please ensure that you and your survey are in compliance with University IRB rules and procedures before initiation of the project. For guidance on this topic, and to avoid violating any UNTHSC or Federal policies, go to the IRB website or contact the Office for the Protection of Human Subjects (OPHS).

A UNTHSC-approved template is provided. Any changes to the template or additions must be approved by the UNTHSC Qualtrics Administrator (Office of Organizational Excellence), in conjunction with the Marketing & Communications department. Also, all surveys intended for all faculty, staff or students must be approved by the Office of Organizational Excellence.

On-campus classroom training is provided by Suzanne Gravois, SystemHR. Support for faculty is provided by the Division of Academic Innovation and can be requested by emailing canvas@unthsc.edu. Support for non-graded academic evaluations (course evaluations) is provided by the Division of Student Affairs Testing and Evaluation Services.
