Course Equivalency Form

The form below may be used to inquire if a particular course will satisfy a prerequisite requirement for the PharmD Program at the University of North Texas System College of Pharmacy. You must indicate which prerequisite requirement you are inquiring about and then provide details of the course you believe satisfies the requirement. A staff member from the Office of Admissions will review the information you submit and give you a determination.
