All students are required to provide documentation of immunizations. A complete list of immunization requirements and FAQs can be found below. Students enrolled in a program that is online-only will be exempt from submitting immunization records. All other students must upload documentation for immunizations and maintain up-to-date records in Medicat. If you are an incoming or current HSC student needing physicals, immunizations, or titers, you can schedule an appointment with the Student Health Clinic by calling 817-735-5051. Students may request an exemption from immunization based on medical contraindications or sincerely held religious practice or belief. View the HSC policy for Immunizations and Health Screenings for Students for details.
If you are seeking an immunization exemption for:

- Medical reasons, complete the Medical Immunization Exemption Request.
- Reasons of conscience, including religious belief or practice, students must request and complete the Texas Department of State Health Services Exemption form Immunizations for Reasons of Conscience Affidavit.
- Non-state required vaccinations (including COVID-19 and influenza) for reasons of conscience, complete the Religious Immunization Exemption Request.
Exemption requests must be submitted to studenthealth@unthsc.edu for review. Students who request an exemption will be notified in writing if the request was approved or denied. View the HSC Procedure Manual of Student Immunization Exemptions for more information regarding the exemption process.
